- Findings
- The Alleged Victim (AV) relied on the facility for care and safety. According to an investigation, on or about December 25, 2025, AV left the facility without staff knowledge. AV did not return to the facility for approximately 24 hours, and staff had no idea how AV left or returned, placing AV at risk for harm. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care, and constitutes abuse as defined by Oregon Administrative Rule 411-020-0002(1)(b)(A)(ii).
