Findings: The Alleged Victim (AV) relies on the facility for their care. AV is a high fall risk with a history of falls. AV is known to walk away from their walker has a history of misjudging distances when sitting. AV is care planned for staff to provide full assistance with walking needs, to remind AV to use assistive devices and if seen without walker, staff is to sit AV down and get the walker for AV. According to an investigation on or about December 22, 2025, AV experienced a fall in the dining room. AV stated AV tried to sit down and missed something. AV was seen twenty minutes before the fall walking towards the dining room table. The facility failed to provide appropriate services, relating to care planning, following the care plan and/or implementing appropriate interventions, posing a risk of harm to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
