Findings: The Alleged Victim (AV) relies on the facility for their care. AV is considered high fall risk with a history of a medical condition that results in collapse. AV is care planned not to be left unattended in their wheelchair in any area of the community and to be taken to their room to be assisted to the restroom after each meal. AV is care planned both as independent and stand by assist for mobility and walking. According to an investigation, on or about January 2, 2026, AV was found alone in AV’s bathroom standing at the sink with a head injury. Prior to the incident, AV was last seen in the dining room, sitting in AV’s wheelchair after lunch. The facility failed to provide services, relating to following the care plan and providing clear direction regarding the delivery of services in the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
