Findings: The Alleged Victim (AV) relies on the facility to manage their medications, including reordering medications in a timely manner. According to an investigation, the AV was not administered their psychotropic medication on or about 12/2/2025, 12/3/2025, 12/4/2025 and 1/5/2026 because it had not been reordered and was not available. The AV experienced headaches and increased anxiety as a result. The allegation that Alleged Perpetrator 2 AP2 and Alleged Perpetrator 3 AP3 neglected the AV was investigated and determined to be inconclusive. The facility failed to maintain a safe medications system and administer AV’s medication as ordered, which is a violation of resident’s rights, is neglect of care, and constitutes abuse.
