Findings: The Alleged Victim (AV) relies on the facility for their care. AV has a history of falls and is known to have a shuffling gait and trip over their own feet. According to an investigation, on or about January 9, 2026, AV experienced a fall in the common area of the facility, resulting in a laceration to their forehead and bruising. The facility failed to provide appropriate services, including care planning and/or implementation of interventions, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
