Findings: The Alleged Victim (AV) and Witness #1 (W1) have a history of altercations. Interventions included keeping W1 away from others when appearing agitated and to monitor AV when near W1. On or about January 12, 2026, W1 yelled at and pushed AV. Interventions include escorting AV past W1's room, deter residents from standing in W1's doorway and consulting W1's Physician regarding medication to manage W1's behaviors. Respondent failed to ensure supervision to ensure the safety of AV, placing AV at risk for harm. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(ii).
