- Findings
- The Alleged Victim (AV) relies on the facility for care and services. AV suffered multiple noninjury falls from January 3 thru January 15, 2025, and the facility failed to implement any additional intervention to try and mitigate AV from falling. The facility failed to add new interventions to AV's service plan, which is a violation of resident rights and is neglect which constitutes abuse.
