Findings: The Alleged Victim (AV) relies on the facility for care and services. According to an investigation, Witness 1 (W1) and AV shared a room together. W1 was known to have agitation and other altercations with other facility residents. The facility did not follow up on progressive interventions for W1's behavioral symptoms, which led AV to getting hurt by W1. The facility failed to implement interventions for W1, which led to AV getting injured in an altercation with W1, which is neglect of care and constitutes abuse.
