Findings: Witness #1 (W1) is known to have episodes of anxiety and agitation, triggered by other residents being near his/her room, which leads to altercations. On or about January 17, 2026, the Alleged Victim (AV) was standing in the hallway when W1 yelled at AV and then put his/her hands on AV's chest, pushing him/her, causing AV to stumble backwards. AV was not harmed, however, AV was placed at risk of harm. Respondent failed to ensure the safety of residents. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(ii).
