Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. According to an investigation, Witness 1 (W1) and AV have a history of resident-to-resident altercations. W1 and AV had a verbal altercation that turned into a physical altercation, injuring AV. The facility failed to follow the service plan to keep the residents separated from each other, which is a violation of resident rights, is neglect of care which constitutes abuse.
