Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. On 1/24/206 Witness 1 (W1) wandered into AV's room and AV began to scream. When the facility staff arrived, they found that W1 had covered AV's mouth with their hand to quiet AV down. W1 is service planned for one-to-one supervision in the facility. The facility failed to follow the service plan for W1, which allowed W1 to go into AV's room and caused AV to experience distress. The facility's failure to follow the service plan is a violation of resident rights, is neglect of care which constitutes abuse.
