Findings: The Alleged Victim (AV) is a known fall risk with a history of falls. Facility documentation indicated the AV had fallen on October 17, 2025, October 18, 2025, October 20, 2025, December 7, 2025, and December 19, 2025. According to an investigation, AV had another fall on or about December 20, 2025, and was sent to the emergency department and diagnosed with a punctuated subarachnoid Hemorrhage. The facility failed to provide appropriate services according to the4 Alleged Victim’s needs, relating to mitigate falls and the risk of injury due to falls, which is a violation of resident rights, is neglect of care, and constitutes abuse.
