Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment and provide care. On February 3, 2026, AV was let out of a secure door by a visitor who did not realize AV was a resident. The facility staff called the police, and AV was found and returned to the facility by the police. The facility failed to provide a safe environment and provide care for AV, which is a violation of resident rights, is neglect of care which constitutes abuse.
