Findings: The Alleged Victim (AV) has had previous falls with injury. Interventions put into place are not effective to reduce AV's falls. On or about January 22, 2026, and January 24, 2026, AV suffered falls without injury, however, AV was placed at risk for harm. Respondent failed to ensure proper care planning to reduce the risk of falls. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
