Findings: The Alleged Victim (AV) relies on the facility for their care. According to an investigation, on or about January 26, 2026, AV experienced a fall. AV requested pain medication approximately twenty-six times between 1/26/2026 and 2/2/2026. AV reported soreness multiple times, requested additional assistance with transfers and complained of pain. AV was not seen by a medical professional until on or about February 2, 2026. The facility failed to ensure timely medical treatment, which is a violation of resident rights, is considered neglect of care and constitutes abuse as defined in OAR 411-020-0002(1)(b)(A)(i).
