Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. Witness 1 (W1) and AV have had multiple residents to resident altercations, and W1 has a history of grabbing AV. Between 2/6 and 2/9/26 AV ad W1 had multiple resident-to-resident altercations, which left AV at risk for serious harm. The facility failed to implement and act on providing a safe environment by ensuring that AV and W1 had adequate interventions in place to ensure safety, which is a violation of resident rights and is neglect which constitutes abuse.
