Findings: The Alleged Victim (AV) relies on the facility for their care. AV is care planned as independent with toileting, requires standby assistance for showers, and requires physical assistance with personal hygiene and grooming. AV was known to have frequent accidents when toileting, struggled to get to the bathroom, and would smear AV’s own waste. According to an investigation, AV went to the doctor multiple times for continued eye complications and on or about January 6, 2026, diagnosed with a bacterial eye infection. The facility failed to provide appropriate services, to include care planning, implementing interventions and provide or assist with proper hygiene, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
