Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. According to an investigation, AV and Witness 2 (W2) had multiple altercations on or about 02/11/2026. The facility was care plans stated that both AV and W2 were to remain in the line of sight for staff to monitor when using the smoking area. The facility failed to have enhanced monitoring for AV in the smoking area, which allowed AV and W2 to engage in physical altercations. The facility failure to provide a safe environment is a violation of resident rights, is neglect which constitutes abuse.
