Findings: The Alleged Victim (AV) has cognitive deficits and relies on the facility for his/her care. According to an investigation, on or about December 31, 2025, a laundry room door was left propped open and AV wandered through the doorway into the garage area of the facility. Staff noticed AV in the garage area and AV lost their balance, resulting in a fall with fractured hip. The laundry room door was supposed to remain closed at all times and the garage area was not intended to be accessible to residents. The facility failed to provide a safe environment which is a violation of resident’s rights, is neglect of care and constitutes abuse.
Sanction: RCFCP26-00560 $500.00 fine assessed
