Findings: The Alleged Victim (AV) requires reminders and escort to all meals and is dependent on the facility for providing all food and nutritional support. According to an investigation, the AV was identified to have weight loss. The AV lost 15 pounds, or 14.72% of their body weight, in six months. There was no documented evidence of the implementation of interventions or notification of the AV's primary care provider to address the significant weight loss. The facilities failure to address the AV's weight loss is a violation of resident’s rights, is neglect of care, and constitutes abuse.
