Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. The AV was able to leave the secure courtyard of the facility because the rear gate was left open and was able to go into the front on the facility and was observed by staff and brought back inside of the facility. The facility failed to provide AV with a safe environment but not ensuring that contractors and staff made sure the gate remained secure, which is neglect of care and constitutes abuse.
