- The Alleged Victim (AV) relies on the facility to protect them from financial abuse. According to an investigation, AV discovered $40.00 missing from their wallet on or about 02/27/2026. The facility failed to protect AV from financial abuse, which is a violation of Oregon Administrative Rule. Alleged Perpetrator 2 (AP2) actions are a violation of resident rights, is neglect and constitutes abuse.
