- Findings
- On or about March 2, 2026, an Unknown Alleged Perpetrator 2 (AP2) failed to provide appropriate services according to the Alleged Victim's (AV) care needs as outlined in AV's care plan which resulted in AV experiencing unreasonable discomfort and suffering bruising. AP2's actions caused AV harm which is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment for AV which is considered a violation of Oregon Administrative Rules.
