Findings: The Alleged Victim (AV) relies on the facility for his/her care and has a history of falls. According to an investigation, the AV had documented falls on March 6, 2026, resulting in a skin tear, and March 8, 2026. On or about March 11, 2026, the AV fell and was identified to have a head injury. The facility failed to appropriately service plan and implement reasonable interventions to address AV’s ongoing falls, which is a violation of resident rights, is considered neglect of care, and constitutes abuse.
