Findings: On or about March 20, 2026, the Alleged Victim (AV) was walking by Witness #1's (W1) room when W1 came out of his/her room and pushed AV to the ground and kicked AV's walker away from him/her. W1 experiences agitation and anxiety from others being near his/her room and has been involved in multiple altercations. Interventions in place to keep others away from W1 is not working. AV was not harmed in this incident; however, AV was placed at risk for harm. Respondent failed to ensure a safe environment for residents. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(ii).
