Findings: On or about March 24, 2026, the Alleged Victim (AV) needed his/her humidifier filled and contacted facility staff for assistance. Staff filled AV's humidifier and AV found that their blood pressure and oxygen levels were unusually low. AV realized that his/her canula did not have oxygen flowing through it and the tubing needed replaced. Staff were not able to change out AV's tubing so AV did it themselves, although staff are responsible to change out the tubing. Some available staff were not trained properly to change out the tubing. AV was visibly distressed and was caused unreasonable discomfort through this process. Respondent failed to provide service and ensure facility staff were properly trained on the care of residents. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
