Findings: The Alleged Victim (AV) relies on the facility for his/her care. AV requires staff to provide repositioning assistance every two hours. According to an investigation, on or about April 13, 2026, a new wound was found on AV's upper back and reported to medical staff. At the time of the incident, AV's service plan instructed staff to reposition AV and log the turning every two hours. Staff had not been turning AV consistently as required and there were no interventions for staff to provide when AV refused repositioning, resulting in AV developing an open wound. The facility failed to provide necessary medical treatment, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
