Findings: The Alleged Victim (AV) relies on the facility for his/her care. The AV has a history of falls and is a high fall risk. According to an investigation, between February 5, 2026, and March 29, 2026, the AV sustained multiple documented falls. On April 20, 2026, the AV was found with injuries to their head and right elbow, blood was observed on the floor. The facility failed to ensure a safe environment and to develop and implement interventions to mitigate the risk of injury due to AV's increase in falls, which is a violation of resident rights, is neglect of care which constitutes abuse.
