Findings: The Alleged Victim (AV) relies on the facility to provide care and services. Av suffered a fall on 3/3/26 and the facility did not have any documentation in AV's service plan for the change of care needs based on the injury AV sustained from the fall. Prior to the fall, AV required little care and after the fall, AV required full assistance from the facility staff, however no changes were made to AV's care plan to alert staff of the change in care needs. The facility failed to update AV's service plan to ensure AV got proper care from the facility staff, which is a violation of resident rights and is neglect which constitutes abuse.
