Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. AV and Witness 2 (W2) were both in the smoking area on the morning of 02/11/2026 when they have an argument which led into a physical altercation. The residents have a history of verbal altercations in the smoking area prior to this day. Later the same day, AV and W2 were both in the smoking area and there was another altercation between AV and W2, which resulted in the top of AV's hand sustaining a bruise. The facility failed to implement interventions to keep AV safe, which is a violation of resident rights and is neglect which constitutes abuse.
