Findings: The Alleged Victim (AV) relies on the facility for their care and is care planned for total physical assistance with incontinence care at routine intervals during the day and night, checking for wet and/or soiled briefs. According to an investigation, on or about May 10, 2026, AV was found sitting in a heavy soiled brief, resulting in unreasonable discomfort. The facility failed to follow the care plan, which is a violation of resident rights, is neglect of care which constitutes abuse as defined in OAR 411-020-0002(1)(b)(A)(i).
