Findings: The Alleged Victim (AV) relies on the facility to administer his/her medications. According to an investigation, on or about May 30, 2026, through June 5, 2026, AV did not receive fluid retention medication as ordered. The medication order had not been entered correctly in the system and not administered, resulting in the AV being treated at the hospital for fluid retention related symptoms. The facility failed to provide a safe medication system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
