Findings: The Alleged Victim (AV) and Witness #1 (W1) have a history of altercations. AV and W1 are care planned to be kept apart to avoid altercations. Staff are to provide stand by assist when W1 is seen wandering in the hallways to prevent altercations. Staff saw AV and W1 approaching each other but did not intervene, as staff stated they did not know they were to be kept away from each other. On or about June 21, 2026, W1 pulled AV's hoodie, causing AV to fall. AV reported pain to his/her left hip. Respondent failed to ensure staff read and followed the service plan to ensure the safety of AV and W1. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
