Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. AV is serviced planned as someone who exit seeks and elopement risk. Alleged Perpetrator 2 (AP2) went through the door that leads to an unsecured area and did not look back to see if the door is closed. AV went through the unsecured door and was found out in the community by staff approximately and hour later. The facility failed to provide a safe environment for AV, which is a violation of resident rights and Oregon Administrative Rule. AP2 failed to provide a safe environment for AV, which is neglect and constitutes abuse.
