Findings: Description of Problem/Complainant statementC reports that s/he was recently told by W1 that RV appeared to have a medical condition that causes confusion, around middle of June 2014.C says that W1 told staff at Somerset ALF several times about the potential medical problem and it took the facility approximately 2 weeks to finally get a urine sample.W1 told C that RV was not like herself and W1 was worried about RV having an infection. During the 2 weeks it took to get the urine sample, RV had a fall that resulted in broken ribs, which occurred at the facility. APS does not have a self report of injury.The fall is said to have occurred around 06/24/14.C reported to APS screener in person 08/28/14 230PM, as she just received the information herself, from W1.) AllegationFailure to assess and intervene in a timely manner for a medical condition that leads to confusion, and RV was at risk of harm. RV had a fall during the time that s/he was not treated for this medical condition.
