This failure has been ongoing from December 1, 2022 to December 31, 2022, and ODHS sent a letter of determination to you. The failure continued from January 1, 2023 to January 31, 2023, and ODHS sent a Civil Money Penalty to you, which was received on February 28, 2023, which reminded you that if you did not report, by March 1, 2023, you would receive further penalties. Failure to report for the month of February 2023 has resulted in further penalties. The facility has failed to comply with this request for the month of February 2023, resulting in a Civil Penalty.
