- The facility failed to consistently staff to the levels, intensity and qualifications indicated by the Acuity-Based Staffing Tool (ABST). The facility was not consistently staffing per the posted staffing plan for two of three shifts on 12/08/24. An investigation determined this is a violation of Oregon Administrative Rules.
