Findings
Based on observation and interview, conducted during a site visit on 06/25/25 and 06/26/25, the facility's failure to have exit door alarms was substantiated. That placed residents at risk and constituted an immediate threat to residents' health and safety. On 06/24/25, the Department received a Report of a Serious Event in which Resident 1 had eloped from the facility through an exterior courtyard door. The facility was a memory care and had the following five distinct and separate neighborhoods: Mount Scott; Happy Valley; Milwaukie; West Linn; and Oregon City. In an observation of the Mount Scott neighborhood on 06/25/25, the alarm of the interior exit door to the courtyard was inaudible. In an interview, Staff 2 (Assistant Executive Director) stated s/he didn't know why it would be turned off, but was able to turn the alarm back on. In an observation of the West Linn neighborhood on 06/25/25, the alarm of the interior exit door to the courtyard was inaudible. In an interview, Staff 4 (Med Tech) stated "the battery is dead." In an interview, Staff 3 (Maintenance) stated s/he fixed the alarm in the West Linn neighborhood. In an observation of the Oregon City neighborhood on 06/25/25, the alarm of the interior exit door to the courtyard was found to have a very faint and fast "chirp" sound when the door was opened. The chirp was faintly audible standing in the doorway and could not be heard in other areas of the neighborhood. In an interview, Staff 3 stated s/he fixed the alarm in the Oregon City neighborhood. The facility's failure to have exit door alarms was substantiated.
