Findings: Based on interview and record review, conducted during a site visit on 02/05/25, the facility’s failure to provide records to the Department upon request was substantiated. Findings include, but are not limited to: A review of electronic communication showed the Department requested documents related to the facility's APS ABST alleged event date of 11/29/24 on 12/12/24. The facility did not provide all requested documentation by the requested due date of 12/13/24. In an interview on 02/05/25, Staff 1 (Executive Director) acknowledged the records were not provided to the Department in a timely manner. The findings were reviewed with and acknowledged by Staff 1 on 02/05/25. The facility’s failure to provide records to the Department upon request was substantiated.
