Findings: Based on interview and record review, conducted during a site visit on April 17, 2025, the facility’s failure to ensure fire drills and staff training were conducted according to the Oregon Fire Code (OFC) was substantiated. Findings include, but are not limited to: A review of fire drills and fire and life safety instruction from September 2024 through March 2025 indicated the following: - Evacuation drill was conducted on 09/27/24. - Unannounced fire drills were conducted on 10/23/24, 12/20/24, and 02/28/25. - There was no documentation of fire and life safety instruction to staff In an interview with Staff 1. The facility failed to ensure fire drills and staff training were conducted according to the Oregon Fire Code. The facility's failure to provide a safe environment is a violation of Oregon Administrative Rules.
