Findings: Based on interview and record review, conducted during a site visit on 06/27/25, the facility’s failure to provide records to the Department upon request was substantiated. Findings include, but are not limited to: A review of electronic communication showed the Department requested documents related to the facility's APS ABST alleged event date of 11/28/24 on 12/19/24. The facility did not provide the requested documentation by the requested due date of 12/20/24. An email communication on 06/30/25, with Staff 1 (Executive Director) acknowledged that the request for records was received and had not been provided to the Department. The facility’s failure to provide records to the Department upon request was substantiated.
