Findings: Based on interview and record review, conducted during a site visit on 04/07/25, the facility's failure to ensure only individuals with a diagnosis of dementia who need support for the progressive symptoms of dementia for physical safety, or physical or cognitive function may reside in a memory care community was substantiated for 1 of 1 sampled resident (#1). Findings include, but are not limited to: A review of Resident 1's service plans dated 01/29/24 and 04/07/25 had not listed a diagnosis of dementia. In an interview with Staff 1 (Executive Director) they stated Resident 1 had not had a diagnosis of dementia. Resident 1 was transferred from the hospital to the facility in January of 2024. The facility's failure to ensure only individuals with a diagnosis of dementia who need support for the progressive symptoms of dementia for physical safety, or physical or cognitive function may reside in a memory care community was substantiated.
