- Findings
- Based on observation and interview, conducted during a site visit on 03/24/25, the facility’s failure to provide an exit door alarm or other acceptable system for security purposes and to alert staff when residents exit the RCF was substantiated. Findings include, but are not limited to: - The facility had two exterior exit doors located on the east and south side of the main floor. The exterior exit door on the south side of the building was observed to have a keypad and magnetic lock system, but there was no alarm system connected to this door. In an interview on 03/24/25, Staff 1 stated the east exit door had an audible alarm due to a resident eloping from it, but the south side door the resident had not attempted to elope from and that was why there was no alarm on it. Staff 1 stated s/he reviewed the alert system for the time of the resident elopement and there were no alert notifications of the east exterior exit door being forced open. In an interview on 03/24/25, Staff 3 it was stated that s/he had not seen an exit door alert come up on the facility’s call system. The facility failed to provide an exit door alarm or other acceptable system for security purposes and to alert staff when residents exit the RCF which is a violation of Oregon Administrative Rules.
