Findings: Based on interview and record review, conducted during an investigation on 07/07/25, the facility’s failure to provide records to the Department upon request was substantiated. Findings include, but are not limited to: A review of electronic communication, dated 06/04/25, indicated the Department requested documents related to the facility’s Acuity-Based Staffing Tool (ABST) alleged event date 05/30/25. The facility had not provided all requested documentation by the due date of 06/05/25. An interview with Staff 1 (Executive Director) acknowledged that the facility had received the Department's request for documentation and had not provided the documents. The facility’s failure to provide records to the Department upon request was substantiated.
