- Findings
- Based on interview and record review, conducted during a site visit on 06/16/25, the facility’s failure to document that they have observed and evaluated the individual’s ability to perform safe medication and treatment administration unsupervised was substantiated. This posed an immediate jeopardy situation which put the health and safety of residents at risk. The facility’s failure is a violation of Oregon Administrative Rules.
