- Based on interview and record review, conducted during a site visit on 06/16/25, the facility’s failure to ensure adequate professional oversight of the medication and treatment administration system was substantiated for 1 of 1 sampled residents (#1). This posed an immediate jeopardy situation, which put the health and safety of residents at risk. The facility’s failure is a violation of Oregon Administrative Rules.
