Findings: Based on interview and record review, conducted during an offsite review on 04/07/25 the facility's failure to implement an Acuity-Based Staffing Tool (ABST). The facility failed to have appropriate staffing based upon their ABST on February 4, 2025, however, there is no consistent information to determine that the facility consistently did not staff appropriately, therefore, no condition placed at this time. The facility's failure to staff to their ABST is a violation of Oregon Administrative Rules.
