- Findings
- The facility failed to have a fully implemented and updated ABST was substantiated. In an interview, Staff 1 (Interim Administrator) stated with the management change they were in the process of updating posted staffing plan to reflect the recent ABST hours for proper staffing levels. The facility’s failure is a violation of Oregon Administrative Rules.
