Findings: In an interview with Staff 1 (Asst. Administrator) stated that wall pendants light up a board at the reception area (does not make a sound) and neck pendants go to a pager however, on 05/24/25 a staff member had taken the pager home, and the pager was not available to staff that were working. Staff 1 stated they are now using a "check in and check out" for the pager system for each shift, and stated Prestige will be adding additional pagers. It was confirmed the facility failed to consistently provide a call system that connected resident units to the care staff center or staff pagers. The facility’s failure is a violation of Oregon Administrative Rules.
