Findings: The facility's posted staffing plan did not exceed the ABST and had not been updated. Based on interview and record review, conducted during a site visit on 06/16/25, the facility's failure to fully implement and update an Acuity-Based Staffing Tool (ABST) was substantiated for 1 of 1 sampled resident. A review of Resident 1’s call light history report on 05/24/25 indicated the resident had called for assistance starting at 6:45 pm. Resident 1 kept calling for assistance until 8:06 pm when staff responded. Resident 1 waited for assistance for an hour and 21 minutes. It was determined the facility's failure to fully implement and update an ABST was substantiated. The findings of the investigation were reviewed and acknowledged by Staff 1 and Staff 2 (Regional Director of Operations). An investigation determined a licensing violation had occurred. The facility’s failure is a violation of Oregon Administrative Rules.
